This is a post for a full or part-time Consultant Cellular Pathologist.  The successful candidate will join the department to support the provision of a comprehensive histopathology and cytopathology service for the county of Herefordshire, and parts of neighbouring mid-Wales.  The post is based in a purpose-built, fully UKAS-accredited laboratory at the County Hospital in Hereford.   The department has a highly flexible approach to both the scope of the candidate’s skills and to home working.  There are also opportunities to get involved in performing post-mortems.

The department will support a hybrid model of working on site and home; all MDTs can also be covered remotely. 

The department is the lead laboratory in our digital network which includes Worcester Acute Hospital and South Warwickshire Foundation Trust.  We are working towards a model whereby cases can be shared digitally across our network to make maximum value of local reporting expertise. 

The department has biomedical scientific staff who can support a full repertoire of specimen dissection and is in the process of introducing the Macropath dissection system to allow the process to be viewed remotely should the reporting consultant require. 

Main Duties of the Job 

To fully participate as part of the team of consultants in the provision of comprehensive histopathology, non-gynaecological cytopathology, and morbid anatomy services (optional) according to agreed rotas, and to assist in the maintenance and development of those services in accordance with medical and technical advances and within the prevailing budget. 
To participate in appropriate clinical and multi-disciplinary meetings with particular emphasis on those areas in which the post-holders have special interests. 
To participate in local, regional, and national EQA schemes in general histopathology, breast screening, bowel screening and non-gynae cytopathology as appropriate.

Breast outpatient clinics take place in dedicated Breast clinic rooms, which are equipped with ultrasound machines. One stop Breast assessment outpatient clinics are run by three surgeons (including this post), with assistance from Breast Radiologists. 

The Breast reconstructive surgery service is also supported by Plastic Surgeons based at QEH, Birmingham.

This post is a part time (6 programmed activities) Consultant Microbiologist for Wye Valley NHS Trust (WVT). This is a new post increasing the consultant establishment to 3.6 wte.  

WVT is part of a programme board with Worcestershire that is exploring the benefits of a network. Currently, there is a microbiology laboratory on site at WVT.  The ICD role is for WVT only.  

Candidates for the post must be either GMC registered medical practitioners or if applying as a consultant clinical scientist to be registered with the HCPC. It is expected that the appointee will hold the FRCPath or MRCP or show evidence of equivalent training and experience. 

Main duties of the job 

Direct clinical care (DCC) and proactive liaison in a timely manner with consultant colleagues, other hospital staff, medical microbiology colleagues and lead respiratory physicians for tuberculosis management, GPs, and the antimicrobial pharmacist. The sexual health service is outsourced to a private provider. There is a 0.5wte antimicrobial pharmacist. An antimicrobial pharmacist technician supports this post. 
The consultant microbiologists share clinical duties. There is an ITU ward round daily 
The consultant microbiologists attend three MDTs – TB and NTM three monthly: prosthetic joint infections monthly and OPAT weekly. This latter consists of pharmacy and the community nurses. The consultant in charge of the patient retains clinical responsibility. The consultants in infection also alternate attendance at a multi-disciplinary clinic for patients with infected diabetic foot ulcers that occurs every fortnight. 
The consultant microbiologist who leads on antimicrobial stewardship attends the Medicines Safety Committee; chairs the six monthly Antimicrobial Stewardship Committee and meets monthly with the antimicrobial stewardship team. 
The head of department attends pathology board. 
The appointee will take an appropriate share of responsibility for departmental commitments according to interests and expertise.

Clinical practice and liaison 
Timely clinical liaison in response to blood cultures/critical specimen results and/or clinician requests is expected. GUM is outsourced to a private provider with no clinical input from WVT. The consultants in infection share clinical duties. There is an ITU ward round daily.  A Clinical Scientist/Higher Specialist Scientist trainee also supports the department. 

There is a 0.5 wte antimicrobial pharmacist. This post is supported by an antimicrobial pharmacist technician. The AMS program is under continuous development, including development of new antibiotic/infection guidelines, enhanced AMS activities and a new penicillin allergy de-labelling initiative. 

The consultants in infection attend several MDTs: 


TB and NTM – with respiratory physicians (3 monthly) 
Prosthetic joint infection/revision meeting – with orthopaedic department (monthly) 
OPAT meeting – with podiatry, community nursing and pharmacy (weekly). The consultant in charge of the patient retains clinical responsibility. 
Diabetic foot infection clinical – with podiatry, orthopaedic foot surgeons, diabetes specialist and vascular team (fortnightly) 
Clostrididioides difficile round (weekly)

The consultant in infection who leads on antimicrobial stewardship attends the Medicines Safety Committee, chairs the 6 monthly Antimicrobial Stewardship Committee and meets monthly with the antimicrobial stewardship team.  The head of department attends pathology board. 
Infection prevention and control 
There is an infection prevention team consisting of a Band 8a 1.0 wte lead nurse, two band 7 0.8 wte deputies and 3 wte band 6 infection prevention nurses who work closely with the ICD. The DIPC is the Director of Nursing. There are 2 band 4 0.4 wte administrator who supports the team. The budget holder is the lead infection prevention nurse. The ICD is the deputy chair of the infection prevention committee. 

There are 4 PAs for the ICD role which is provided only for WVT 

The ICD attends the decontamination committee, the infection prevention committee, the cleanliness and food safety committee, the water management group, the PFI performance management group and the Herefordshire and Worcestershire-wide HCAI forum. 

The apportionment and possible rotation of roles will be decided between the appointee and the existing medical staff. There are additional opportunities for teaching (including for junior medical staff, students and our Higher Specialist Scientist trainee) and a weekly hospital Grand Round which our department participates in.

Wye Valley NHS Trust’s Department of Haematology is seeking to recruit a  Consultant Haematologist to join the current team and consolidate a sustainable consultant-delivered service to the population of Herefordshire and East Powys. The department offers first class care within high quality, dedicated facilities, delivered by a friendly team which provides a comprehensive range of services up to Level 2a. 

Candidates with wide general training in Haematology are the priority and special skills to support or complement the existing consultants would be welcomed. The department works closely with colleagues at Worcester Acute Trust (where MDT, level 2b and 3 services are centred) and colleagues at our main tertiary centre, the Queen Elizabeth Hospital, Birmingham. 

There is flexibility within the department to accommodate the skills of the successful candidates. Candidates wishing to work up to and including 10 PA contracts are actively encouraged to apply.

Main duties of the job: 

Hands-on clinical involvement as a Consultant Haematologist, working closely with other Consultant Haematologists and  Associate Specialists and sharing responsibility for all haematology patients. 
Participation and attendance at joint MDT with WAT and in the future potential participation in any joint on call / attending system initiated. 
Taking on a lead role in an area of interest. This to include participation on the appropriate Trust committees and lead for education and audit in that area. 
Participate in clinical trials and become Principle Investigator for appropriate studies. 
Liaising and developing working policies with the relevant hospital departments. 
Liaising and developing working policies with local general practitioners, consultant colleagues, nurse practitioners and other relevant health professionals 
Teaching and training students and medical staff at all levels as well as nurse practitioners, nurses and paramedical staff. 
Active participation in clinical audit activities within the Department of Haematology and Trust-wide. 
Maintenance of CPD is expected and there is a trust-wide policy for study leave funding. 
On call (level A – 5% supplement) – 1 in 4 basis currently.
